Hypocalcemia causes tetany as hypocalcemia means decreasing in extracellular calcium which will make the membrane potential on the outside less negative and less amount of depolarisation is required to initiate action potential. This will lead to increase excitability of muscles and nerve tissue by decreasing the threshold needed for neurons activation.

Neurons will become unstable and trigger involuntary contraction of the muscles which finally leads to tetany.
